When Should I Consider Breast Augmentation Revision?

Thorough pre-operative planning, discussion, and execution of breast augmentation surgery will lead to successful results in the vast majority of cases. Changes can occur over time, however, leading to the consideration to undergo revision surgery. Breast implant revision is an option if you have older implants that you wish to have replaced with more modern ones. Implant position or breast fold location may be corrected at this time. Some of the common reasons for revision surgery include:

  • Increasing or decreasing the size of the implants
  • Changing from saline to silicone implants
  • Correcting sagging breasts
  • Addressing the malposition of implants

Complications can also occur at any time, requiring correction. Either the breasts are uneven or asymmetrical or the implant isn’t correctly positioned. Other women notice changes in the shape, size, or volume of their breasts. Changes occur because of aging, weight fluctuations, or stretched skin.

General anesthesia is normally used when removing breast implants. Dr. Colville is generally able to use the original incision to insert the new implant, usually removing scar tissue from the original implant at the same time.

Complex Breast Augmentation Revision

Complex breast implant revision surgery includes removing and replacing breast implants from a previous breast augmentation, often for reasons complicated by other factors. Sometimes patients do not wish to have their implants replaced, so Dr. Colville performs breast implant removal, which may include capsulectomy (removal of the capsule). Revision breast surgery is required for a number of reasons, including:

  • Capsular contracture or implant hardening
  • Implant leakage
  • Implant has changed position
  • Implant rippling and noticeable feel
  • Asymmetry
  • Symmastia
  • Aging breasts droop away from the implants

Improved augmentation techniques can now correct issues that were impossible to correct 10 years ago. Most, if not all, breast augmentation procedures performed decades ago inserted implants directly behind the breast glands, which is called a subglandular placement. This placement often caused problems, especially for the saline breast implants used at the time. Common problems included capsular contracture, implant rippling, and shifting implant position.

Today, Dr. Colville often recommends a submuscular placement for the implants. In this procedure, he inserts the implants not only behind the breast gland, but also under the pectoral muscle. This technique relies on the muscle for more natural coverage, and to better hold the implant in place while reducing the chances of capsular contracture, according to some studies. (The exact causes of capsular contracture are unknown at the present time.)

In some cases of capsular contracture, Dr. Colville recommends revision surgery with Strattice® — a natural matrix that helps support or reposition the implant or minimize the chances of recurring capsular contracture or other complications.

The Strattice tissue matrix is approved by the FDA and is designed to support tissue regeneration. It is made of the same natural products widely used for years to repair and replace damaged tissues in other surgical procedures.

What is Strattice Tissue Matrix?

The tissue matrix is derived from porcine dermis. It’s a soft, natural product designed to reinforce weak tissue and support the growth of your own tissue. Since 2002, products such as the Strattice tissue matrix have been used in post-mastectomy breast reconstruction. They have also been used to treat surgical, trauma, and burn patients for more than 10 years.

First let me start off by saying: Yes, I would do it again! The staff and Dr. Colville are professional and are there to answer any questions/concerns. I was a diabetic and on 3 medications, overweight. So with help from family, I lost weight on my own 80# or more and didn’t like the way I looked. A family member told me about Dr. Colville and staff, so I called, consultation and surgery-a whole body lift! I am off all medications, not a diabetic and I feel wonderful, self esteem is great and I have more confidence in myself. I will tell you that with any surgery there is pain and recovery time but in the end it was well worth it!

Dr. Colville is an extremely talented, thoughtful surgeon. I would say, an artist. I can't say enough good things about my entire experience, from consultation through surgery and healing. He makes you feel like his most important patient and he approaches your case with precision-like accuracy. He didn't "over promise" but he certainly "over delivered." I had a consultation with Dr. Colville at the beginning of January, just to discuss options and price for surgery. I was impressed with the staff and the first person I spoke with in the consultation room. However when the doctor came in I was less impressed. I felt he was trying to talk me out of my desired procedure as soon as he started speaking to me. He was very knowledgeable I’ll give him that but I was uncomfortable with the way he made me feel as far as seeking plastic surgery. Also I don’t feel that he took any of my wants for the outcome seriously and basically decided everything and told me that’s what he was going to do, I didn’t feel I had any say in my own procedure.

Dr. Colville's and his staff were so warm and welcoming from day one. He came highly recommended by many friends and did not disappoint. His consultation was thorough and very knowledgeable. I did not have an implant size in mind, just wanted fuller breasts. He did mention that he can only go as large as the skin was willing to stretch. I was pleasantly surprised, once after surgery, I realized he went a little larger than what he intended to do. My recovery was little to no pain and my incisions healed flawlessly. After all the swelling had gone down my breasts sit perfectly on my chest and I couldn't be happier. I'm able to wear many shirts and dresses with no bra, that I was not able to wear before, and I fill out my bathing suits perfectly.! My strapless bra still slides down, but who needs one when your breasts sit perfectly on their own!! Thank you for your excellent work!!

I had a breast lift and breast implants done by Dr Colville in 2017. I have been debating whether to review him or not but I think everyone should hear the good and the bad to make informed decisions, especially about a major surgery. I loved him and his staff at first. We discussed the size I wanted and I went to surgery. After surgery, I found out he went quite a bit smaller than what we discussed. Soon after I started developing terrible keloid scars on all incisions around my nipples and underneath my breasts. When I expressed concern, Colville replied 'if you don't like them you can have them redone in 1 year'. I was devastated and appalled at his comment and lack of concern. I paid $11k which is a ton of money to Me. This was a huge decision and for him to act like it was a drop in the bucket along with the lack of empathy. I had to do my own research about keloid scars and asked them about steroid injections. They said they could try it and gave Me one set of injections. I gave up after no change and disappointing care. I read later that you have to do multiple weeks of injections to see any change. I felt that they were not knowledgeable about keloid scarring or how to help Me. Now, 3 years later, I still have keloid scars and My Breasts started to sag again after about a year. I don't know if anything could have been done about the smaller size, scarring, or sagging skin but I would have liked them to offer to correct things with at least a major discount or act concerned in the slightest. I was terribly disappointed. At the end of the day, my breasts are definitely better looking than the saggy breasts I had previously after 3 kids, however, due to the scarring that shows through shirts and thin bras I have to be careful what I can wear and how I look...I feel like I went from one problem to another after spending $11k. I hope this helps someone to make an informed decision. Would I get them done again knowing what I know now? Yes. Would I use Colville? Idk but I would have definitely done way more research and would have appreciated honest reviews like this.

When I went to dr colville both him and all of his staff were so incredibly supportive and kind. I really felt that every member of his team was competent in what they were doing. I had wanted a breast augmentation since I was young but was always scared. I saw so many horror stories and bad reviews of other surgeons. Both me and my husband spent a great deal researching the best surgeon for me and we were not let down by dr colville. He addressed all of our questions and concerns. I was able to make as many appointments as I needed and wanted, as well as changes to the implants along the way until I was happy with what we decided. Even with all the extra appointments and emails with questions or changes, not once did him or his staff make me feel as if I was being difficult or annoying. They were patient and understanding with us. I am currently 4 months post op and I am beyond happy with my results. I feel more confident in myself physically and couldn’t ask for a better surgeon or team. I went quite large, under the muscle with galaflex for support. He did a wonderful job with the surgery and dealing with my anxiety before, during and after. I have recommended him as a surgeon to many of my friends looking for a cosmetic surgeon for any reason and will continue to do so.

I took a few years to make the decision to have a breast augmentation. I had considered it many times but after seeing Dr. Colville, I was sure of my decision. Dr. Colville explained things in great detail and put aside many fears that I had. He was kind but also straightforward and answered all my questions honestly. After going through with the procedure and fully recovering, I can honestly say he was right about everything. My recovery process was quick and relatively painless. I was able to go to an event at my daughters school the very next day. I knew exactly what to expect as I healed. It was so much easier than I originally anticipated. It helps having an experienced surgeon who knows what is going to fit well with your body. I highly recommend Dr. Coleville for anyone considering this procedure. If anything, a consultation to get any questions answered you might have. I am very happy with my results. He did an amazing job!

I searched for a year and looked at many before and after breast augmentations and read review after review of all kinds of plastic surgeons. I finally landed on Dr. Colville to be the surgeon that offered everything I was looking for. I knew I wanted a breast augmentation because I was tired of wearing kids bras and looking like a child in a shirt. I was not looking for an overwhelming enhancement but one that made me look like a women when wearing a low cut top or dress. The staff here is nothing less than amazing. Everyone was helpful and makes you feel welcome. Questions were never left unanswered and my augmentation went great. Healing time was important to me because I didn't want to be laid up in bed for months. His method of surgery and recovery is beyond quick and as painless as I think it can come. I recommend him to everyone looking to get plastic surgery as I was very impressed. I wish I would have done it years ago!

Plastic surgery is an elective procedure that comes with risks and benefits. After interviewing 4 other plastic surgeons, I felt that Dr. Colville was the perfect choice for me for several reasons. I'm an athletic 42yo mom of 2 and after losing 110 lbs and keeping it off for over 10 years by continuing to live a healthy lifestyle, it was time for me to make a change to my body that would help transform me into the way I deserved to look. We chose the best option for my body to include abdominoplasty with lipo, and mastopexy with reduction. I searched for the surgeon that I felt I could TRUST and receive the healthiest outcome with. Dr Colville performed my breast reduction surgery in August of 2018 and it was one of the greatest things that has happened to me. I was a size 38I and was absolutely miserable. Back, shoulder and neck pain, rashes under both breasts...shopping for clothes was always a horrible experience, forget about buying a button down shirt, or a tshirt that wasn't a men's size and only being able to purchase bras online....now shopping is fun and I can actually wear women's sizes AND actually go to a store and find bras that will fit!!! Dr Colville and his staff were (and still are) fabulous! I would highly recommend!!!!!

I researched off and on for 10 years about breast augmentation surgeries, implants, scaring, Plastic surgeons (local and even states away) and their credentials. After multiple consults with different plastic surgeons, Dr. Colville was the best choice. Dr. Colville and staff explained everything with zero pressure. The level of professionalism, compassion and attention to detail can not be stressed enough. For me surgery wasn't just about getting a bigger chest size. Being a very athletic petite build, the goal was to look natural and to actually be able to wear shirts and bras not bought from the kids section. After surgery, healing went exactly as explained. It's now been 1 year and thrilled is an understatement. Bra size went from a 32AA to a 32D. The majority of people didn't know I had surgery because it looks so natural and assumed I was training chest more at the gym. The only regret from this entire process is that I waited for so long to do it.

I started my weight loss journey in 2014 at 276 pounds. I used a jogging app on my phone and started seeing results within the first month. After losing 100 pounds I found myself with a lot of extra skin. It was a constant reminder of what I had done to my body and I told myself that if I could sustain my weight loss for several years, that I would get my arms fixed. Four years and one pregnancy later (from which I had to re-lose 60 pounds) I had maintained my weight loss. Like most of you, I have heard great things about Dr. Colville and his staff. When I first scheduled my appointment, it was not up to the standard I expected from his staff. I have to admit I was pretty upset. When I was able to explain to Dr. Colville and his staff the situation and how disappointed I was with my interaction they immediately reached out to me. Not only did a woman, Annette, call me to sincerely apologize but informed me that Dr. Colville would be reaching out personally to apologize. Then soon after I received a very sincere letter from Dr. Colville and his staff. I could not be more pleased with my experience. I spent 2 years researching cosmetic surgeons, and the doctor that kept coming back with the highest ratings, referrals, & credentials was Dr. Colville. Then a close friend of mine told me that she had been to see him and highly recommended him, so I decided it was time to schedule a consultation. I needed an upper blepharoplasty on only my right eye. Which meant that he had to take excess skin from my right eyelid and try to match the left untouched eyelid to help make them as symmetrical as possible. Something I was very nervous about and required a huge amount of trust as your eyes are one of the 1st thing people see. Very difficult to hide if not done properly. Thus the 2 years of research. Let me tell you.... Dr. Colville made me feel very comfortable and confident in the procedure. He told me exactly what to expect and walked me through every step as he was doing it. No surprises. His nurse Robyn knew I was nervous and actually held my hand and soothed me while it was being done. She was fantastic. The results were breathtaking. I couldn’t believe how beautifully he had matched my other eye. Perfection. Dr. Colville is an artist. Extremely precise. Exceeded any expectations I had ever hoped for. I highly reccomend him. He will forever be the only cosmetic surgeon I will ever go to for as long as he is in practice. I would also like to mention that everyone at the office was great as well. Then a close friend of mine told me that she had been to see him and highly recommended him, so I decided it was time to schedule a consultation. I needed an upper blepharoplasty on only my right eye. Which meant that he had to take excess skin from my right eyelid and try to match the left untouched eyelid to help make them as symmetrical as possible. Something I was very nervous about and required a huge amount of trust as your eyes are one of the 1st thing people see. Very difficult to hide if not done properly. Thus the 2 years of research. Let me tell you.... Dr. Colville made me feel very comfortable and confident in the procedure. He told me exactly what to expect and walked me through every step as he was doing it. No surprises. His nurse Robyn knew I was nervous and actually held my hand and soothed me while it was being done. She was fantastic. The results were breathtaking. I couldn’t believe how beautifully he had matched my other eye. Perfection. Dr. Colville is an artist. Extremely precise. Exceeded any expectations I had ever hoped for. I highly reccomend him. He will forever be the only cosmetic surgeon I will ever go to for as long as he is in practice. I would also like to mention that everyone at the office was great as well. Extremely welcoming, friendly, and on top of things. You will be very good hand and well taken care of there from start to finish.

Dr Colville gave me a breast reduction in 2011. It was painful. Which he told me. But my breast turned out a perfect size small D. Instead of off the charts. My neck and back no longer hurt. Hello I am a 41 year old mother i have 3 kids married 20 years. The reason why i went to Doctor Coville was for the past few years i have been talking with my husband about getting breast augmentation of course as husbands says he found nothing wrong but to me after having the kids and me working out 4 days a week i just didn't feel my breast looked the same. I had my consult with him and he did a scan to be able to send me pics with the different size beast he felt would be right for me after talking with him to let him know what i wanted mine to look like and feel like. I wanted them to look and feel real and perky like they use to and also wanted my clothes to fit correctly. I was a little nervous this was a big decision for me to make but he knew exaclty what i was looking for and he and his staff mad me feel very comfortable and i love the end result i was a size A cup i went to C/D cup depending on the Bra. would recommend Dr Coville and his staff if you are thinking of getting breast augmentation done.

I recently had breast augmentation done with Dr Colville and I can not believe how comfortable and easy the process was. His staff felt like family; extremely kind and guided me through the whole process. Dr Colville himself is so talented and helped me decide on a size that would balance out my body the way I wanted. I loved that I was able to have a visual thanks to their imagining system. My surgery went so smoothly and pain level had been at a 2-3 or less while recovering. About an hour after surgery, I was already able to put my arms overhead. I love the shape and size we picked for me! I highly recommend you to start with consultation if you have ANY thoughts or concerns about having breast augmentation done.

I've lived in the Toledo area for 51 years. Over the years, I have had to see many different doctors for various reasons. Dr. Colville is one of the best. Not only is he good at what he does, but he cares about his patients. Several years ago, Dr. Colville was going to give me a neck lift. I was all set and sitting in the operating waiting room when he came in and told me he was up all night thinking about my procedure and he doesn't think it will give me what I'm looking for. He sent me home. How many doctor's think about their patients to that extent? How many doctor's would've done the operation anyways? I can not believe that Dr. Colville put my needs, the patient, in front of his own. A few years back, I decided to try facial aesthetics/injections. Dr. Colville was not as expensive as the other doctor's in the area so I went to him and I am so glad that I did. Dr. Colville is very skilled at what he does and I have not had any regrets. As far as "bed side manner" Dr. Colville's is very down to earth. He does not talk down to me as his patient and he listens without judgement. Although infections are not cheap, he does not charge as much as other plastic surgeons in the area. I have thanked him over and over for not charging astronomical prices for his services. This gives women and men, who are not in the top 1%, a chance to look good and feel great about themselves without taking out a second mortgage. As good as he is, he could be charging double and people would probably still go to him. I recommended Dr. Colville to a friend who needed reconstructive breast surgery and to my sister-in-law who wanted a full face lift. Both women were completely satisfied and glad that they went to him. So, if you are looking for a plastic surgeon in the Toledo area, I feel, you can't go wrong with Dr. Colville.

I have been a 32A for the majority of my life. After having 3 children, my bra size was a 32AA. I had been considering breast augmentation for several years. When I finally decided to go for a consultation, Dr. Colville came highly recommended from several of my friends. I discussed with Dr. Colville that I didn’t know what bra size I was hoping to achieve, I just wanted to look full and natural. I trusted his expert opinion as to what size implants would work best for me. He recommended between 295-310cc’s. The day of surgery went quickly. After surgery, I was sleepy and my chest felt tight, but I didn’t experience any significant pain. Over the next 2-3 days, I took it easy. After about 3 days, I was back to normal, playing with my kids and feeling great. It took several weeks for the feeling of tightness to go away, but the end result was amazing! I can buy bras from actual stores instead of ordering petite sizes online! I ended up getting 295cc’s of silicone on each side and am a 32C, maybe 32D, depending on the brand. I can shop now without ever having to consider if a top will fit me. My confidence is much higher as well. The surgery and recovery were quick with little to no pain and the end result is fantastic! I will never be able to thank Dr. Colville and his staff enough for changing my life and giving me the confidence that I have been missing!

I AM SO PLEASED WITH MY ENTIRE EXPERIENCE FROM MY INITIAL VISIT TO MY FOLLOW UP. IN MY CONSULT WITH DR COLVILLE I HAD STATED TO HIM I WANTED TO HAVE A THIGH LIFT AND A BREAST LIFT. HE TOLD ME THAT A BREAST LIFT WAS NOT NECESSARY AND THEN WENT ONTO EXPLAIN WHY. I WAS IMPRESSED AS HE WAS COMPLETELY HONEST WITH ME AND SAVED ME SOME MONEY. HE THEN WENT ONTO ASK ME WHAT I WAS LOOKING FOR IN MY RESULTS. I TOLD HIM AND HE THEN EXPLAINED TO ME WHAT WOULD NEED TO BE DONE TO GET THE RESULTS I WAS LOOKING FOR. I had a great experience with Dr. Colville. The staff was very upfront with all information that I needed to make a decision about my cosmetic surgery experience. (Tummy tuck and breast augmentation.). I felt they were realistic as to all parts of the process...the surgery, the expectations of what my body will look like and the healing process. The cost was right in line with other surgeons in the area. I liked that I was in a hospital for the surgery. And I liked Bella Via for my post operative massages. All members of the staff were very professional and helped make my experience a very positive one. The best part is that I love my new look!!!

I have been pleased with my experience with Dr. Colville thus far. The staff has been friendly and helpful and wait times have been reasonable. Dr. Colville performed an abdominoplasty for me. My concern was more muscle repair than aesthetics. He acknowledged my reasoning for the surgery and was honest with me regarding the effectiveness of the procedure for me. Other surgeons I had looked into tried to sell me additional cosmetic procedures I was not interested in nor considered necessary. Dr. Colville is very matter of fact but shows concern for his patients and is skilled surgeon.

I have known Dr. Colville for several years and have been to his office for many minor procedures from mole removals to botox to endermologie After much consideration, I finally decided to get a breast augmentation. It's been 5 months and I am so very pleased with the outcome. I knew he would be conservative as I was not looking for overly large breasts. The nurse really helped in the evaluation. Everyone at the office is so nice and professional. My initial consultation with the imaging device was extremely helful in assuring me of the final outcome. I'm amazed at the changes occuring each month. 5 months out and I'm thrilled. He does great work. Thank you to Dr. Colville and his entire staff!

I had a breast augmentation with Dr. Colville about 9 months ago. His staff is very attentive and polite. They always try to fit me in during my lunch breaks. I really came into his office not knowing the size and shape that I wanted, but Dr. Colville is an artist. He was able to look at me and give me a size and shape that are perfect for my body type. I would definitely recommend this surgeon.

Solution to Complications

Dr. Colville can use Strattice tissue matrix to help women who have undergone surgery elsewhere, and have complex breast issues including:

Bottoming out

Bottoming out is a common complaint from women who have had breast implants for some time. Long after their breast augmentation surgery, some women experience a lengthened nipple-to-inframmamary fold distance — in other words, their nipples seem to have moved up on the breast. In reality, the breast pocket itself has dropped lower than desired, which stretches the skin beneath the breast. Breast skin laxity or the loss of skin elasticity may have also caused the breast to drop.

In the revision surgery, Dr. Colville attaches the tissue matrix internally to the inframammary fold and chest muscle, which can help support and position your breasts in the desired location.


Fold malposition

When the fold below your breast (the inframammary fold) or the fold on the side of your breast (the lateral fold) moves, it’s called “fold malposition.” The form and defining borders of the breasts are affected by natural folds that should provide gentle curvatures under and on both sides of your breasts. These folds give the breasts definition and help maintain their shape and position.

After breast augmentation surgery, one or both breast pockets can be compromised by either the loss of skin elasticity or a breakdown of pocket boundaries, affecting either the inframammary fold, lateral fold, or both. Should this occur, the breast pocket may drift to the outside of your chest or appear to have dropped. They will no longer be in their original, desired position.

In breast implant revision surgery, Dr. Colville readjusts the breast pockets and re-establishes the inframammary and lateral folds. He then uses strips of the tissue matrix to redefine the fold location by supporting the fold repair inferiorly or laterally.


Symmastia

Breast implants can sometimes come too close to the midline of your chest, resulting in a condition called symmastia (sometimes called “bread-loafing” or “uniboob”) The natural space that exists between breasts is gone, leaving a woman without cleavage. Whether the breasts are naturally large or have been enhanced, there should be a degree of separation to give each breast definition.

Symmastia can occur following breast augmentation surgery if:

  • The breast pocket created was made too large
  • Your breasts have migrated out of their original position
  • The fibrous tissue that creates the medial fold becomes weak or stretched

The Strattice tissue matrix can help support the repaired or newly created medial fold. During the breast implant revision, Dr. Colville will use the tissue matrix to reinforce existing weak or inadequate tissue, which may help provide additional support to hold the breast pocket in the desired location.


Wrinkling and rippling

Wrinkling and rippling can occur when the skin is stretched too thin or when the breast pocket exceeds the optimum size needed. Also, if your breast implants migrate out of their original position because the breast pocket is too large, the appearance of rippling may occur. Rippling also commonly results when breast implants are too large and they “bottom out.”

The quality of the skin that envelops the breast tissue significantly influences the shape of your breasts. Even though your breast skin contains special elastic fibers, there is natural and hereditary variation in the amount of elasticity and thickness of each person’s breast skin, and this affects the overall appearance of your breasts.

Dr. Colville places the tissue matrix at the bottom of your breasts to reinforce weak or inadequate tissue. This can help support and position your breasts in the desired location, reducing the presence of wrinkles and ripples on the skin surface.


Capsular contracture

Capsular contracture is an unpredictable condition that may occur when scar tissue that naturally forms around your breast implants continues to increase in thickness. As thickness increases, it squeezes the breast, causing tightness, firmness, and changes in the appearance of your breasts. The condition may occur in one or both breasts and may vary in degree and severity. Discomfort and changes in breast appearance may require additional surgery. There is currently no known cause for this condition. The only known solution is to remove the capsule and replace the implants, sometimes in a newly created pocket.
